Evaluation Forms

To best prepare for the performance evaluation process, the following forms should be completed as needed.

During the performance cycle:

In order for an employee to receive an overall “extraordinary contribution” rating, he or she has to have received an extraordinary contributor form DURING the performance cycle. Please do not wait the performance evaluation is due to give the employee the form and submit it to HR for the personnel file.

On a similar note, an employee can not receive an overall “below contributor rating without having received a “Notice of Improvement Needed” form during the cycle. To obtain this form, please contact Judy Kwitnieski at jkwitnie@umw.edu.

Self Assessments:

NOW is the time for classified employees to complete an optional self assessment. Employees can use the form linked above which allows them to insert the core responsibilities from their current EWP, a form created by the supervisor, or they can choose to write a narrative. Whichever form is chosen, employee input on the following factors is helpful:

  • General accomplishments and obstacles during the performance cycle
  • Achievement of the core responsibilities/measures of their position;
  • Implementation of the performance factors (interpersonal relations, communications, safety, attendance, etc…)
  • Learning points from this cycle; and
  • Desired goals and training for the new cycle.

Employees should get this to their supervisors as soon as possible so it can be reviewed before completing the evaluation.

Use the current EWP to rate each core responsibility, special assignments, and applicable performance factors, taking into consideration any submitted employee self-assessment.  Assess the employee’s progress toward meeting his/her learning goals.  Determine the overall performance level.  This action requires your judgment and objective assessment of the employee’s total job performance throughout the review cycle.

Employee Development plan:

Use this time to outline some development opportunities for your employees. This helps establish some clear goals and lets them know you care about their career advancement.  Using the Employee Suggestion Form will help include input from your employee to their goals and plans in their position.
