This form is for Permanent Parking reservations only. If you would like to temporarily reserve a space/lot for an event please use the TEMPORARY RESERVATION REQUEST FORM.

Availability of permanent reserved spaces is limited and requests are granted only in special circumstances
Once the request has been received by Parking Management, it will be submitted to the department VP for approval
