iPads may be purchased with Wi-Fi only option using this standard form. If you require the 3G/4G option, please submit your request on a Non-Standard Equipment Request form.

AppleCare+ is OPTIONAL when purchasing iPads. After selecting which model iPad you want to purchase, please select the AppleCare+ option you want.

***If you do not select one of the AppleCare warranty selections below, Procurement will assume you DO NOT WANT AppleCare and the additional warranty will NOT be purchased.***
